How to reserve

Patients who wish to undergo dialysis treatment at our centre for a holiday period must contact the Nephrology and Dialysis Centre, in order to agree on the period and number of dialysis sessions to be performed, through one of the following methods:

• by telephone Monday to Friday from 9:00 to 16:00 at: (+39) 0173-316.579
• by Reservation Form (Modulo 1) downloadable from our site, sent by fax or e-mail.

Once the reservation is accepted, the affiliated Dialysis Centre must fax or e-mail the dialysis treatment form and clinical report (Modulo 2), downloadable from the site.

For Italian patients it is necessary to bring a health service authorization to the Centre, in which the type of treatment and the number of sessions to be performed are specified.

Non-national patients must bring their Health Card to the Centre.
